I can only answer the one of those questions as I've had that situation. The ladybugs should take care of the aphids (if that is what they are) all by themselves. I had a ton of ladybugs all over my chiles and tomatoes this past summer and I had aphids in the beginning of the summer, right after I transplanted the plants into the raised bed. I saw quite a few all throughout the summer. Never saw anymore bugs. I did see 2 hornworms and 2 stink bugs and that was it the whole summer. So, good luck with your aphid situation. I hope the other pod-heads answer the other 3 questions. I think that I had a ladybug website somewhere that showed pictures of the larvae. If i find it, I'll pass it on.
