About the only place I know of that will really fire up the food 
(that I can think of right now) is in Bethesda Maryland and is called 
the Cottonwood Cafe. You can get there by Metro but it's a pretty 
good hike from the station to the resturant.

You have to convince them that you really like it hot, but once done 
they will get crazy.  My favorite dish there is the Habanero Duck, 
although Fire and Spice run a close second. The appatizers are also 
great.

There is also a Thai resturant called Terra Thai in the same general 
area which is pretty good but most of the time the wait can be over 
an hour.
