By anyones' standards, Blairs' Sudden Death is considered a VERY hot sauce (might be my Savina(tm) in there :-). If you look, you will see that it and his After Death both use pepper extract. Any sauce using pepper extract is going to be pretty darn hot. Blair has also made a very limited amount of another sauce called "2 AM" (his private reserve- 600,000! Scoville) which is probably TWICE as hot as his 'Sudden Death'. Uses include, but are not limited to, winning bar-room bets, bringing people that brag that 'nothing is too hot' to their knees, and getting even with various in-laws & out-laws. Culinary uses include as an additive to stews, soups, and other such. It certainly isn't designed to be eaten straight, although there are always some few people who can.
