Of course disease comes -- and eventually the plant dies, but this doesn't happen until I've gotten loads of tomatoes. I know that my plants won't make it from Febuary to December (our frost free times) -- but I just plan on two crops, and a time during the worst weather where I'm reduced to buying tomatoes from the local farmers market.
